The In’s and Out’s: Internal V.S. External Rehabilitation for Patellar Tendinopathy

Introduction: Patellar tendinopathy (Also known as patellar tendinitis, or PT) is a relatively common soft-tissue injury that results in pain and inflammation of the tendon inferior to the patella. PT is common in athletes who play sports with excessive jumping and/or landing (basketball, volleyball, etc.) PT is caused by a variety of factors such as poor flexibility, muscle weakness, and the most common reason, overuse. Exercise is the best intervention for PT, however, athletes with patellar tendon pain often have trouble performing the exercises needed to recover fully. Intrinsic rehabilitation methods, such as slow eccentrics and single-leg isometrics, can address the pain at the source, and extrinsic methods, such as infrapatellar straps and Kinesio-tape®, can temporarily relieve pain.
Focused Clinical Question: Are internal or external rehabilitation techniques more effective in aiding in relief for athletes with patellar tendinopathy?
Search Strategies and Results of Search: Search terms included patellar tendinopathy, patella tendinopathy, jumper's knee, kt tape or kinesio tape or kinesiology tape, infrapatellar strap, Isometrics, Eccentrics, Rehabilitation or rehab, and athlete or athletes or student athletes. Databases utilized were PubMed, Medline, and Health and Medicine.
Key Evidence and Results: a well-structured intrinsic rehabilitation program is necessary to fully recover from patellar tendinopathy; however, external factors such as infrapatellar straps can be used to provide short-term relief in athletes who are active regularly/currently in season.
Application / Implication: Athletic trainers should utilize rehabilitation techniques to help athletes treat pain and address both short and long term goals for recovery. Coaches can adjust training for individuals with PT both in season and out of season to assist in full recovery.
